Life Beyond School - Successful Integration into the Post School Environment
Little By Little (with VISY funding) has been able to do a lot or work with our families to support our students as they are leaving school. We run information sessions for families, visit service providers, organise transition experiences with our students and then follow them into their chosen services. Since we started this program in 2011, our students leaving school and moving to a successful placement has been 100%. Prior to implementing this program success levels sat at 60% of students. Our model has been shared with other schools and services providers at local and state levels.
